This Giveaway is sponsored by ATI Holdings, LLC (“Sponsor” or “ATI”).
Eligibility:
Giveaway participants must be (i) a U.S. resident, excluding residents of Florida and New York, (ii) at least 18 years of age at the time the giveaway begins, and (iii) a student physical therapist (SPT) currently enrolled in a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited or accredited pending Doctorate of Physical Therapy program at a university within the United States or hold an active Physical Therapy license within the United States. ATI’S APTA-CSM Giveaway (“Giveaway”) is void outside the United States, or where prohibited or restricted by law.
All employees, officers, directors and agents and their immediate families or any members of their households, regardless of relationship, of Sponsor or any advertising agency or other company involved with the design, production, execution or distribution of this Giveaway are not eligible to participate in the Giveaway or to win any prizes.
Any Giveaway entries received from an ineligible participant will be disqualified. By participating in this Giveaway, participants accept and agree to be bound by these rules and the decisions of Sponsor, which shall be final on all matters. Sponsor reserves the right to modify the rules of the Giveaway in any way at any time, subject to state or federal law.
Giveaway Period:
The Giveaway begins on August 1, 2025 at 12:00 am, and ends on November 6, 2025 at 12:00 pm (“Giveaway Period”). All entries must be received and recorded by Sponsor during the Giveaway Period to be eligible.
How To Enter:
A participant can enter the Giveaway by visiting https://therapy.atipt.com/csm2026/ and following the on-screen entry instructions. The participant must fill in all required information in order to enter. Each participant may enter only once. Use of any robotic, automatic, programmed, or similar entry method or entering more than the number of times permitted will void all entries and result in disqualification. You may not enter more times than indicated by using multiple email addresses, identities, or devices in an attempt to circumvent the rules.
Sponsor is not responsible for lost, late, unsent, incomplete, illegible or unintelligible entries or for inaccurate entry information, whether caused by the participant or by any of the equipment or programming associated with or utilized in this Giveaway, or by any technical or human error that may occur in the transmittal of any entry or the processing of any function on the participant’s computer system. Participant must notify Sponsor of any changed contact information within the Giveaway Period to remain eligible to win based on entry information. Sponsor reserves the right, in its sole discretion, to disqualify any individual found to be tampering with the entry process or the operation of the Giveaway; to be acting in violation of these rules; or to be acting with intent to annoy, abuse, threaten or harass any other person. Any use of robotic, automatic, programmed or similar response methods will void all such entries submitted by such methods.
Prizes and Selection of Winners:
Giveaway participants will enter for a chance to win one of three all-expense paid trips to APTA CSM 2026, February 12-14, 2026, in Anaheim, CA. For each winner Sponsor will cover the winner’s CSM registrations costs, transportation (as selected by Sponsor) costs to and from the event, and lodging (as selected by Sponsor) for three nights from February 11, 2026 through February 13, 2026. The approximate prize value for each winner is $1,800.
The chances of winning depend on the number of entries received. No transfer, assignment, cash redemption or substitution of any prize is permitted, except by Sponsor due to prize availability and then with a prize of equal or greater value.
The winners will be selected in random drawings on September 5, 2025, October 3, 2025 and November 7, 2025. All non-winners at each drawing date will be entered in any remaining 2025 drawings. Sponsor will notify the winners individually by email.
The winner must respond via email to APTA-CSMGiveaway@atipt.com within five (5) days of being notified or the prize may be forfeited and another Giveaway participant chosen. In the event that the potential winner is disqualified for any reason, Sponsor may select an alternate potential winner by random drawing from among all remaining eligible entries.
Winners are solely responsible for all applicable federal, state or provincial, and local taxes and any expenses associated with any prizes. The Internal Revenue Service requires Form 1099-MISC to be issued and filed for each recipient who received a prize/award if the annal aggregate is at least $600 or more. Winners who are residents of the United States of America must complete and submit a W-9 form to receive any prize. The recipient is responsible for any income tax liability and should consult their tax advisor for the proper tax treatment of this income on their tax return.
Sponsor reserves the right, in its sole discretion, to modify, terminate, or suspend this Giveaway should viruses, bugs, unauthorized human intervention or other causes beyond Sponsor’s control corrupt or impair the administration, security or fairness of the Giveaway. Sponsor is not responsible for any human or technological error that may occur in the processing of the Giveaway nor are they responsible for any error, omission, interruption, deletion, defect, communications line failure, theft or destruction of, unauthorized access to, delay in operation, transmission, or distribution of, or alteration of entries, whether caused by the sender, any of the equipment or programming associated with or utilized in this Giveaway, or otherwise.
Publicity:
Except where prohibited, participation in the Giveaway constitutes each winner's consent to Sponsor's and its agents' use of winner's name, likeness, photograph, voice, opinions and/or hometown and state for promotional purposes in any media, worldwide, without further payment or consideration.
Limitation of Liability:
By entering the Giveaway, participants release Sponsor and its affiliated companies, directors, officers, employees, representatives, partners and agents from any liability whatsoever for any claims, costs, injuries, losses or damages of any kind arising out of or in connection with the Giveaway or with the acceptance, possession or use of any prize (including, without limitation, claims, costs, injuries, losses or damages related to personal injuries, death, damage to, loss or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light).
Dispute Resolution:
Entrant agrees that all matters arising out of or relating to this Giveaway and these Official Rules are governed by, and construed in accordance with, the laws of Illinois, without giving effect to any of its conflict of laws provisions thereof. Entrant further agrees that any legal suit, action, or proceeding arising out of or relating to this Giveaway and these Official Rules shall be brought exclusively in the applicable federal or state courts located in Chicago, Illinois.
